INVESTIGATIVE REPORT
On February 12, 1998, SAA. L. Strope, in furtherance of the investigation into the death of Lisa McPherson, began to collect information from the case file that would indicate that Alain
Kartuzinski and Janis Johnson had control over Lisa’s watch.

In a statement taken from Alain Kartuzinski on December 4, 1995, he admitted transporting Lisa McPherson from Morton Plant Hospital to the Scientologist’s Ft. Harrison Hotel in Clearwater.

In her initial statement, Janis Johnson admitted that she was requested by Alain Kartuzinski to become involved with Lisa McPherson.

On April 22, 1997, Dr. David Minkoff testified at deposition as a result of a State Attorney’s subpoena.

Dr. Minkoff said that he had several conversations with Janis Johnson and Alain Kartuzinski concerning drugs that were given to Lisa McPherson (pages 15-25, deposition transcript).

Dr. Minkoff testified that Alain Kartuzinski called requesting chioral-hydrate for Lisa McPherson (page 87, lines 1-15, deposition transcript).

Dr. Minkoff testified concerning Janis Johnson’s ability to care for Lisa. He said, “Well, I knew she was a medical doctor. She was... she had been in practice. I mean, she was a professional.
She, I thought, could examine a patient and had an understanding of the process that was going ( )on, so I thought that it would be safe to do that with her there” (page 88, lines 23-25 and page 89,
lines 1-3, deposition transcript).

Dr. Minkoff was asked by SA Strope, “Was it your understanding that Dr. Johnson was capable of delivering that medical examination?” He replied” In my conversations with her initially, yes, I determined that she was” (page 104, lines 11-14, deposition transcript).

On May 16, 1996, Dr. Jeannie Decuypere testified at deposition as a result of a State Attorney’s subpoena.
